    Skate Canada Collection of Personal Information Statement (Appendix A)

    Your privacy and the protection of your personal information is important to us. Your personal information is required to register you with Skate Canada in any capacity, including, without limitation, as a Skate Canada registrant, coach or in connection with your affiliation with a skating club or skating school and to administer various services, such as Skate Canada events. Your personal information may also be exchanged with Skate Canada affiliates which includes your local skating club or skating school or provincial association or section. By submitting this form, you expressly provide your consent to the sharing of your personal information with Skate Canada and as described herein for purposes of registration and receipt of national services delivered by Skate Canada. We adopt the 10 Fair Information Principles into our privacy program, and employ reasonable measures to protect against unauthorized access, processing, disclosure, alteration, destruction or loss of your personal information.     CanSkate Pricing:
    $275.00 One Day per week FALL or WINTER
    $345.00 One Day per week both FALL and WINTER
    $375.00 Two Days per week FALL or WINTER
    $495.00 Two Days per week both FALL and WINTER

    NEW - Parent & Tot
    (Toddler, PreCanSkate, Stage 1 & 2)
    ​Parent & Tot CanSkate is a program developed to allow a family member to assist in their young skater's development. This program allows the skaters to receive hands-on assistance from a family member who they know, trust and is in their cohort.  
    This program introduces the fundamental skating skills such as balance and glide by using fun interactive games and activities.  Parents & Tots will follow the CanSkate curriculum which is divided into 6 stages. Each of these stages are divided into Balance, Control and Agility.  Adults and skaters will participate in small group lessons led by an NCCP certified professional coach. Upon mastering the skills in this program, skaters will move into our CanSkate program.   The adult and the skater both must wear a CSA approved hockey helmet.      Both the skater and the family member must pay the $45.00 fee to be registered with Skate Canada (included in the fee).

    Note: Parents/Adults must be 15 years of age and have basic skating skills (balance on two feet, move forward and stop).

    Please Read the following Policies and Waivers carefully:
    WAIVER: The applicant (Skater listed above or legal Guardian if Skater is under 18) agrees to release the Huntsville Skating Club, its officers, directors, coaches, and members from any claims for damages arising from any accident, injury, or loss which is caused by or arising from participation in any program or event offered by the Huntsville Skating Club. Further the applicant agrees to abide by the Huntsville Skating Club code of conduct and rules and regulations of Skate Canada and the Huntsville Skating Club.
    HELMET POLICY: All skaters in the CanSkate Program must wear a CSA certified Hockey Helmet while participating in Huntsville Skating Club on ice activities at all times until the skater has passed CanSkate Level 6. Face shield / cage is optional.

    All participants in the Youth PowerSkating Program must wear a CSA certified Hockey Helmet including face shield / cage at all times on ice.

    During special events, such as Santa Skate, where registered skaters, friends, and family are participating on ice the following will apply: All skaters must wear a CSA certified Hockey Helmet unless they have already passed CanSkate Level 6 and are also currently registered and insured with the Huntsville Skating Club. This means that PARENTS must also follow our helmet policy otherwise they will be unable to participate. Face shield / cage optional.
    CANSKATE RULES: A Parent is Required to be Visible in the arena to care for child should they need to exit the ice. Certified CSA Hockey helmet is mandatory by Skate Canada.  Skaters are not to enter the ice until instructed by a Coach or Program Assistant. No Parents / Spectators are permitted inside players benches, penalty boxes, time keepers box, or media box. Parents / Spectators are not permitted to open any ice door without consent from a Coach or Program Assistant.
    EMAIL POLICY: The Huntsville Skating Club uses email as the primary form of communication and distributing information to our members. The Huntsville Skating Club will use any email address provided in this application for this purpose. 
    PHOTOGRAPHY POLICY: The Huntsville Skating Club may use any photograph, video, or likeness of any participant in Huntsville Skating Club programs, events, or activities for promotional or fundraising purposes. All skaters or participants can choose to remain off camera provided that the Huntsville Skating Club is notified in writing (letter or email) stating the name(s) of all individuals who shall not appear on camera.
    Covid-19 Waiver

    via email PRIOR to stepping into the facility.

    I acknowledge that I am the skater participating in physical activities at the (insert name of Skating Club/Skating School and I have read the Government of Ontario Concussion Awareness Resource for my age group and the Skate Ontario Concussion Code of Conduct and understand what a concussion is and how it may be caused. I understand the importance of reporting a suspected concussion to my coaches and my parent/guardian. I understand that I must be removed from practice/competition if a concussion is suspected. I understand that I must provide written clearance from an appropriate health care provider to my coach before returning to practice/competition. I understand the possible consequence of returning to practice/competition too soon and that my brain needs time to heal.

    ​I acknowledge that my child is participating in physical activities at the [insert name of Skating Club/Skating School) and I have read the Government of Ontario Concussion Awareness Resource for his/her age group and the Skate Ontario Concussion Code of Conduct and understand what a concussion is and how it may be caused. I understand the importance of reporting a suspected concussion to coaches and any other designated persons at the club/skating school. I understand that my child must be removed from practice/competition if a concussion is suspected. I understand that I must provide written clearance from an appropriate health care provider to my coach before my child is permitted to return to practice/competition. I understand the possible consequence of returning to practice/competition too soon and that my child’s brain needs time to heal.

    • E-transfer is the preferred method of payment, please send to recipient ​huntsvilleskatingclub@gmail.com                                            If you are unable to utilize that method of payment, please contact the club to arrange another method of payment.
    All receipts for payment will be provided upon request to the club email huntsvilleskatingclub@gmail.com.

    NSF Cheques
    A $35 service charge will be applied to all NSF cheques. Cash, certified cheque, money order, or email money transfer will be the only methods of payment for individuals who have issued NSF cheques in the past.
    All CanSkate & Youth PowerSkate Program fees include the $45.00 Skate Canada fee.
